Registration

Please bring the following when you register your child:

1.                  Legal Birth Certificate  (must be 5 years old by Dec. 2, 2011)

  • (Can be obtained at the County Clerk’s office)
  • (No Hospital Certificate Accepted)

2.                  Proof of residency (Utility bill / receipts of parents)

3.                  Shot record which includes the following:

a.                  Polio: 4 doses, or 3 with the last one given after the fourth

birthday.

b.                  DPT: 5 doses, or 4 with the last one given after the fourth birthday.

c.                  MMR: 2 doses given after the first birthday.

d.                  Hepatitis B: 3 doses required

e.                  Varicella (Chicken Pox): One dose (or physician verified disease)

(**All Birthday references refer to 4 (four) days before the stated birthday.)

No child will be allowed to register / enroll unless their immunizations are complete.
